Sickness & Misadventure Form

In the event a student is unable to attend an examination due to illness, a positive COVID-19 test, misadventure, or unforeseen family circumstance, please complete this form and attach any medical certificates, notifications from external agencies, or other relevant supporting documentation.

This also covers students who attend the exam but are unable to finish due to sudden onset of illness. This will allow teachers to calculate a standardised mark based on the part of the exam completed.

PLEASE NOTE: A medical certificate or other documentation supporting notice of illness or an event preventing examination attendance must be provided. Without the form and documentation, the exam will be resulted as zero, and may compromise the completion of the unit of work.
